Terms & Conditions

Effective date: October 2024

The terms governing use of the RISE Physio & Wellness website, booking tools, clinic communications, and general appointment and payment policies across our Shelburne and Orangeville clinics.

These Terms and Conditions explain the use of RISE Physiotherapy’s website, booking tools, and clinic communications, together with general appointment and payment policies.

“RISE,” “we,” “us,” and “our” refer to RISE Physio & Wellness, operating as RISE Physiotherapy at our Shelburne and Orangeville locations.

These terms do not replace the informed consent process for assessment or treatment, our Privacy Policy, or your rights under applicable law.

01

Our Services

RISE provides physiotherapy assessments, treatment, rehabilitation, and related services as described on our website and discussed with your practitioner.

Services, practitioner availability, appointment types, and fees may vary between locations. Please confirm that you have selected the appropriate clinic, service, and practitioner when booking.

Treatment recommendations depend on your assessment, clinical needs, goals, and practitioner’s professional judgment. Outcomes and recovery times vary, and we cannot guarantee a particular result.

02

Website Information and Medical Advice

Our website provides general information about our clinic and services. Website articles, videos, exercises, and other educational materials are not a substitute for an individual assessment or advice from a qualified health professional.

Do not begin or change treatment solely because of information on this website. Ask your practitioner whether an exercise or treatment is suitable for you.

Visiting our website or submitting an enquiry does not, by itself, establish a practitioner–patient relationship or constitute consent to treatment.

Our website, general contact forms, email, and text messages are not emergency services. For a medical emergency, call 911 or attend the nearest emergency department.

03

Appointments and Online Booking

Appointments are subject to availability and confirmation through our booking system or clinic team.

Please provide accurate contact information and review your appointment confirmation for the location, date, time, practitioner, and appointment type.

Keep your booking account credentials confidential and notify us if you believe your account has been accessed without authorization.

We may occasionally need to reschedule an appointment because of practitioner illness, unexpected closures, or other circumstances. We will make reasonable efforts to notify you and discuss available alternatives.

04

Cancellations, Missed Appointments, and Late Arrivals

Please contact us as soon as possible if you need to cancel or reschedule.

  • Cancellation notice required: 24 hours prior to your scheduled appointment.
  • Late cancellation and missed appointment fee: A standard clinic fee applies to late cancellations (less than 24 hours notice) and missed appointments, subject to exceptional circumstances or illness.

Any cancellation or missed appointment charge will be disclosed before it applies. Such charges are separate from treatment fees and will not be represented as treatment received.

If you arrive late, your appointment may need to be shortened or rescheduled to avoid delaying other patients. Any applicable charge will follow the policy communicated to you in advance.

Please contact the clinic if illness, an emergency, or another exceptional circumstance affects your attendance.

06

Patient Responsibilities and Respectful Conduct

To help us provide appropriate care, please:

  • Provide accurate information about relevant symptoms, medical history, medications, and changes in your health.
  • Tell your practitioner about discomfort, concerns, or unexpected responses to treatment.
  • Ask for clarification if instructions or recommendations are unclear.
  • Treat clinic staff, practitioners, and other patients respectfully.
  • Follow reasonable clinic safety and infection-prevention procedures.

You remain free to make decisions about your care, including declining recommendations.

Threatening, abusive, discriminatory, or unsafe behaviour may require us to pause an appointment or take other appropriate action. Any decision to discontinue ongoing care will respect applicable professional obligations, including reasonable notice and continuity of care where required.

07

Fees, Payments, and Insurance

We will make applicable fees and payment arrangements available before services are provided. Please contact us if you need clarification about the cost of an assessment, treatment, report, or other service.

  • Payment timing and accepted methods: Payment is due at the time of service. We accept debit, credit cards (Visa, Mastercard), e-transfer, and direct billing through supported extended health benefit insurers.

Insurance coverage varies. You are responsible for checking your plan’s requirements, limits, referral requirements, and eligibility.

Where direct billing is available, it is subject to your authorization and insurer acceptance. Direct billing does not guarantee coverage. We will explain your responsibility for any unpaid balance, subject to applicable funding arrangements and law.

Receipts will accurately describe the services provided and payments received. Please contact us promptly about a billing error or disputed charge.

If prepaid services or packages are offered, their conditions and refund arrangements will be disclosed separately and will comply with applicable legal and professional requirements.

08

Email and Text Communications

Where offered and with the appropriate consent, we may send appointment confirmations, reminders, scheduling updates, and other service communications by email or text.

Promotional messages require separate consent where applicable. Providing a telephone number or booking an appointment does not automatically enrol you in promotional messaging. Promotional consent is not a condition of receiving treatment.

Message frequency varies. Your mobile provider’s message and data charges may apply.

You can change your communication preferences by contacting us. Promotional emails will include an unsubscribe option. Where our text service supports STOP commands, you may reply STOP to unsubscribe; otherwise, use the instructions provided or contact the clinic.

Unsubscribing from messages does not cancel an appointment. Contact us separately to cancel or reschedule.

Message delivery can be delayed or fail. Please verify appointment details through your confirmation, booking account, or the clinic. Do not rely on email or text for urgent care.

For communication assistance, contact hello@risept.ca or 519-306-1655.

09

Privacy and Confidentiality

Our Privacy Policy explains how we collect, use, disclose, retain, and protect personal information and personal health information.

Please avoid sending detailed medical information through general website forms, ordinary email, or text messages. Contact us to arrange an appropriate way to share sensitive information.

Consent to treatment, consent to optional marketing, and consent to particular uses or disclosures of information are addressed separately where required.

For complete details on our information practices, please review our Privacy Policy.

10

External Platforms and Links

Our website may link to external booking, payment, educational, or other services.

External services may have their own terms and privacy notices. Review those notices when using them. We do not control the content or availability of independent websites.

Our use of external providers does not remove our own applicable privacy or professional responsibilities.

11

Website Content and Acceptable Use

Website text, branding, photographs, graphics, and other materials belong to RISE or their respective owners or licensors.

You may view and print materials for personal, non-commercial use. Any exercises or educational materials remain subject to the guidance in Section 2.

Other reproduction, redistribution, modification, or commercial use requires the relevant owner’s permission, except where permitted by law.

You must not misuse our website, attempt unauthorized access, interfere with its operation, submit malicious content, or use forms to send unlawful or abusive material.

12

Website Availability and Responsibility

We make reasonable efforts to keep website information accurate and the website available. However, information may occasionally contain errors or become outdated, and technical interruptions may occur.

Please confirm important details, including appointment availability, fees, and location information, directly with us.

Nothing in these terms excludes or limits liability that cannot lawfully be excluded or limited, waives responsibility for negligent clinical care, or restricts your statutory rights or ability to raise a concern with a professional regulator.

13

Changes to These Terms

We may update these terms to reflect changes in our website, services, or legal requirements. The current version will appear on this page with its effective date.

Changes will apply prospectively. Material changes affecting an existing appointment or payment arrangement will be communicated appropriately, and additional agreement will be obtained where required.

14

Governing Law

These terms are governed by the laws of Ontario and the applicable federal laws of Canada, subject to any mandatory legal protections that apply to you.

If a provision is found unenforceable, the remaining provisions will continue to apply to the extent permitted by law.

Nothing in these terms prevents you from exercising a legal right or making a complaint to a relevant regulator or public authority.
